When STEMI is not STEMI: HIV Myocarditis Mimicking ST elevation Myocardial Infarction on Electrocardiogram: A Case Report
Myocarditis is a common occurrence among human immunodeficiency virus (HIV) infected patients, which either resolves spontaneously or results in death. Anecdotal reports have shown that myocarditis can mimic acute myocardial infarction (MI) with chest pain and electrocardiographic (ECG) abnormalities. We present a case of HIV myocarditis unusually presenting with transient ST-segment elevations and cardiogenic shock.
Philippine Heart Association-Acute Coronary Syndrome Registry- 2 Year Results
An acute coronary syndrome (ACS) registry was initiated by the Philippine Heart Association and launched in November 2011, enrolling 1,939 patients for its preliminary 2-year enrollment period.
Prevalence of Subclinical Right Ventricular Dysfunction Using Tissue Doppler Imaging Among Newly Diagnosed Obstructive Sleep Apnea (OSA) Patients in St. Luke’s Medical Center
Obstructive sleep apnea (OSA) is a syndrome characterized by repetitive obstruction of the upper airway during sleep, and is associated with frequent episodes of transient nocturnal pulmonary hypertension and consequently, possible right heart dysfunction. The aim of this study is to determine the prevalence of subclinical right ventricular (RV) dysfunction in newly diagnosed OSA patients.
Recurrent Tachyarrhythmia in a 50-Year Old Male: A Case Report
Ebstein’s anomaly is characterized by displacement of the posterior and septal leaflets of the tricuspid valve. The most common presentation of this anomaly in adulthood is arrhythmia. This is a case of a 50-year-old male who presented with recurrent arrhythmias in a three-year span.
Association of Chromosomal 22q11 Deletion with Conotruncal Heart Defects in 0 to 18 Year Old Filipino Children
Chromosome.22q11 deletion is associated with DiGeorge syndrome in which conotruncal heart defect is an important feature. Studies showed that non-syndromic patients with conotruncal anomalies may have 22q11 deletion. This study aimed to determine the frequency of chromosome.22q11 deletion, and to describe the clinical manifestations of Filipino patients aged 0 to 18 years with non-syndromic conotruncal heart defects (CTHD).
Effectiveness of Ultrafiltration in the Treatment of Acute Decompensated Heart Failure: A Meta-Analysis of Randomized Controlled Trials
Acute decompensated heart failure (ADHF) is defined as the new onset or recurrence of gradually or rapidly developing symptoms and signs of heart failure requiring urgent or emergent therapy and resulting in hospitalization. Ultrafiltration emerges as a promising alternative to diuretics in patients with signs and symptoms of volume overload. It allows extracorporeal removal of plasma from the whole blood across a semi-permeable membrane through a transmembrane pressure gradient. The objective of this meta-analysis is to compare the benefit of ultrafiltration vs intravenous loop diuretics in patients with ADHF.
A Meta-analysis on the Effects of Tolvaptan in Patients with Chronic Heart Failure
Vasopressin levels are often elevated in heart failure patients and left ventricular dysfunction, and this may contribute to the increase in systemic vascular resistance via stimulation of the V1a receptor. Among the recent novel therapies for heart failure are the vasopressin receptor antagonists. Tolvaptan is an oral, non-peptide, selective vasopressin V2-receptor antagonist that causes aquaresis by acting on the distal nephron to increase secretion of electrolyte-free water. This meta-analysis aimed to test the hypothesis that tolvaptan is beneficial and results in clinical improvement in the treatment of heart failure.
The Use of Intraaortic Balloon Pump Therapy for Intractable Ventricular Tachyarrhythmia
The intraaortic balloon pump (IABP) is the most widely used circulatory assist device in critically ill cardiac patients. This has been useful in patients with incessant and hemodynamically unstable ventricular tachyarrhythmias.
Outcomes of Intravascular Ultrasound Guided-Percutaneous Coronary Intervention versus Conventional Percutaneous Coronary Intervention: A Meta-analysis of Randomized Controlled Trials
Coronary angiography has been the standard test in evaluating coronary artery disease. However, this procedure has its limitations in estimating disease severity. Intravascular ultrasound (IVUS) promises to be a more sensitive tool for correct assessment of true lumen dimensions and architecture, as well as atherosclerotic plaque burden and vessel wall abnormalities. The use of IVUS during percutaneous coronary intervention (PCI) may therefore optimize the results of PCI, particularly of stent implantation. In addition, IVUS provides the operator with better information on real vessel size, which facilitates device selection.
